Move all Claude-specific logic (1355 lines) from scanner.mjs into src/harness/adapters/claude.mjs implementing HarnessAdapter interface. scanner.mjs becomes a thin 10-line re-export wrapper. Co-Authored-By: Codex (GPT-5.5) <noreply@openai.com>
