feat: enhance scanner, server, and UI + streamline AI_INDEX

Expanded scanner with new detection capabilities, added server endpoints,
improved dashboard UI. Trimmed AI_INDEX.md to routing manifest format.
Fixed SKILL.md to use @latest for npx.
